Not for Louisiana residents. Louisiana law affects three of this bundle’s four documents: the Last Will and Testament Kit, the Advance Directive Kit, and the Durable Power of Attorney Kit are each restricted for Louisiana residents, for three different legal reasons. The HIPAA Authorization is unaffected. If you live in Louisiana, talk to a Louisiana-licensed attorney about those three documents instead of using this kit.

What’s inside:

Before you sign:
Each of these four documents has its own execution requirements, and they don’t all match. Some need two witnesses, some accept or require notarization, and the Durable Power of Attorney and Advance Directive have state-specific steps in a handful of states. Read each document’s own instructions before signing it, and give copies to the people named in them once signed, a document nobody can find doesn’t help anyone.
