What this document does:

• Names an Agent to handle your financial and legal affairs, banking, bills, property, contracts, if you become unable to handle them yourself.
• Stays in effect even if you become incapacitated, that’s what makes it “durable,” unlike a standard power of attorney.
• Lets you name a successor Agent in case your first choice can’t serve.

What’s inside:
A grant of authority naming your Agent and successor Agent, a list of the specific powers you’re giving them (banking and financial accounts, real estate transactions, tax matters, insurance, retirement benefits, and more, you can limit which ones apply), and a choice of when the authority takes effect, immediately upon signing, or only if a doctor determines you’re incapacitated. Signature lines and a notary acknowledgment, most states require this document to be notarized before it can be used, especially for real estate transactions.
